Clinical Case Manager

The Clinical Case Manager is responsible for the direct provision of rehabilitation and support services to participating consumers. Case Manager’s visit consumers and provide services in the home, community, or program site.

Clinical Solutions Incorporated is a Community Based Mental Health organization of licensed committed experts that offer a wraparound approach to treatment. Specializing in On-Site and Off-Site Counseling Services & Psychiatric Rehabilitation Service that include: Individual and Group therapy, Parenting Consulting Classes, Substance Counseling, Special Education Consulting, and Mentoring.

A successful candidate will have the following minimum qualifications:

  • Associates degree in human services or related field. Bachelor’s Degree preferred in Social Work, Psychology or Sociology.
  • Or are currently enrolled in one of the above fields of study.
  • Experience in community-based services.
  • Must possess a valid driver license in state of residence and current auto insurance.
  • Clean driving history.
  • Be able to pass an FBI background check.
  • Strong communication and written skills.
  • Highly organized and detail oriented.
  • Strong knowledge of and genuine respect for youth and adults with mental health issues and a firm commitment to empowering their families.
  • Demonstrated ability to work effectively with internal and external individuals, including other professionals in the community.
  • Demonstrated ability to work effectively as a member of a team.
  • Effectively communicate to various internal and external audiences in both person and through various electronic media.
  • Effectively manage multiple priorities simultaneously.
  • Daily traveling within Montgomery County, MD.
  • Frequently traveling into a variety of community environments, including visiting homes that may be unsanitary and/or in need of repair.

Responsibilities:

  • Initial face to face visits with parents or legal guardians of children accepted into rehabilitation program, providing the agency and program overview, and completing all necessary documentation needed prior to the initiation of services. Including case management and entitlements paperwork and all consent and release forms.
  • Initial face to face visits for children and adults accepted into rehabilitation program, providing the agency and program overview, and completing all necessary documentation needed prior to the initiation of services. Including case management and entitlements paperwork and all consent and release forms as well as Code of Maryland Regulation forms.
  • Clinical Case Manager will develop and/or implement the Individual Rehabilitation Plans (IRP) based on the specific strengths, skills and needs of clients on assigned caseload.
  • Clinical Case Manager will meet with collaborative team and update IRP to reflect new success and challenges every 90 days.
  • Clinical Case Manager will complete assigned in-home and community-based visits for each client on assigned caseload as scheduled each month (6 visits of one hour per month for clients over the age of 18 & 4 visits of one hour for clients under the age of 18). It is the expectation for each PRP counselor to meet the requirements stated above.
  • Clinical Case Manager will provide a projected monthly schedule to supervisor.
  • Clinical Case Manager will report to the office at least one hour four times a week. If failed to no show three consecutive times without notification will result in termination.
  • Clinical Case Manager will implement both on-site and off-site rehabilitation activities outlined in the IRP for assigned clients.
  • Clinical Case Manager will provide individualized, create assistance and support designed to help clients achieve their identified goals.
  • Clinical Case Manager will provide and/or coordinate transportation to individual and Rehabilitation group activities as needed.
  • Clinical Case Manager will initiate contact with inter and intra agency professional to coordinate services for clients on assigned caseload.
  • Clinical Case Manager will collaborate monthly with assigned therapists to ensure continuity of care.
  • Clinical Case Manager will maintain and ensure appropriate and timely communication with other mental health professional working with clients on caseload and ensure an efficient coordination of services.
  • Clinical Case Manager will ensure timely and accurate completion of all documentation including but not limed to service tickets, contact notes, crisis plans, incident reports, monthly progress summaries, and quarterly IRP reviews.
  • Clinical Case Manager is responsible for informing supervisor of any engagement issues with clients before billing period in order for engagement to be addressed and continuity of care be continued.
  • Clinical Case Manager will complete all monthly responsibilities and ensure the accurate and timely submission of all documentation and paperwork necessary for Clinical Solutions to submit and receive payment for services. Clinical Case Manager will submit completed check out documents, including signed billing to payroll and supervisor no later than close of business day (COB) 6PM on the 15th of each month. Clinical Case Manager will submit completed time sheets no later than COB 6PM on the 30th of each month. If the 30th falls on the weekend Clinical Case Manager will submit time sheets last business day of month.
  • And any other duties assigned.

Mental Health Therapist

The therapist is responsible for providing counseling/psychotherapy, for Clinical Solution patients with mild to severe mental health illness. The therapist will work to address other social and mental health needs of patients.

A successful candidate will have the following minimum qualifications:

  • Completion of an accredited program with certification and licensure as LMSW (Licensed Mater Social Worker), LGPC (Licensed Graduate Professional Counselor), LCPC (Licensed Clinical Professional Counselor), or as an LCSW-C (Licensed Clinical Social Worker Clinician)
  • Maintain an understanding of how behavioral health priorities are administered under the state of Maryland DHS guidelines, consult with specialists and colleagues to review and participate in the care of patients receiving indirect/contract medical services.
  • Must possess current Maryland licensure as a LMSW, LGPC, LCPC or LCSW-C.
  • Must be able to meet and maintain current Clinical Solutions credentialing and privileged requirements.
  • Must be willing to work evening and weekend hours if necessary.
  • Maintain confidentiality.
  • Maintain HIPAA compliance.
  • Follow license criteria when licensure applies.
  • Follow federal, state and local legal guidelines.

Responsibilities:

  • Managing a caseload of authorized clients.
  • Provide psychotherapy for mild to severe clients who struggle with mental health in various settings (school base, on site, offsite and or virtual). This is determined at the time of initial employment.
  • Completion of initial assessments, record notes and treatment plans of all clients that the provider is currently servicing.
  • Obtaining Information about community mental health needs and resources that could be used in conjunction with therapy.
  • Plan outreach and referral activities in surrounding communities.
  • Plan and conduct programs to prevent substance abuse or improve community health and counseling services.
  • Assess clients’ mental conditions based on review of client information, interviews, observations, tests, and collaboration with other staff members.
  • Serve as a client advocate to coordinate required services and/or resolve emergency problems in crisis situations.
  • Develop and implement treatment plans based on DA, clinical experience, and knowledge.
  • Conduct initial family assessments and refer patients, clients, or family members to community resources or specialists, as necessary.
  • Maintain confidentiality of records relating to clients’ treatment.
  • Location of providing services is on site, off site, school base and virtual unless directed elsewhere by the program manager.
  • Assess the mental and behavioral health status of referred patients.
  • Formulates short-term and long-term counseling plans for patients under care.
  • Consults with medical director and other providers regarding complex patients and regarding prescriptions medications prescribed by providers.
  • Identify patients with urgent mental health conditions (e.g. suicidal, homicidal, psychotic, etc.) and make appropriate referrals to available emergency or psychiatric treatment facilities.
  • Provides group and family therapy session as needed.
  • Participate in a team management approach to patient car/treatment.
  • Maintain appropriate documentation of clinical and counseling services, including all necessary billing information. That includes entering notes in HER/EMR system in a timely fashion.
  • Maintain and update professional knowledge and proficiency through continuing education, staff meetings, and workshops.
  • And any other duties assigned.
